St. John Hospice Mission Statement
The mission, belief and values of Ascension Health and St. John Health, provide the foundation for St. John Hospice Program. The program provides a compassionate approach to palliative care that recognizes the uniqueness of each individual. Hospice addresses the physical, emotional, educational, and spiritual needs of terminally ill patients, their caregivers and families. An interdisciplinary approach to pain management, symptom control, support and quality care will enable the patient to fully live each day.

Hospice provides a compassionate approach to health care that specifically addresses the physical, emotional, educational, social, and spiritual needs of terminally ill patients, their caregivers, and families when curative measures may no longer be an option. Individualized care is provided by an interdisciplinary team whose focus is to provide pain management, symptom control, support, quality care, and a sense of worth and dignity. Specifically, Hospice strives to enable patients to live their lives fully and we provide opportunities for patients, caregivers and families to better communicate and work towards resolving any unfinished issues they may be experiencing.
This team approach has each discipline working together to provide comfort and care to the patient, caregiver and family.

Hospice Services
Comprehensive services are provided to address the needs of the patients, caregivers, and families, 24 hours a day, seven days a week, in their home or place of residence regardless of race, age, color, creed, religion, gender, sexual orientation, nationality, disability, diagnosis, marital status, resuscitation status, availability of a primary caregiver, or ability to pay. Hospice believes that no individual receiving services should feel isolated or alone and that our presence will make a difference to every patient, caregiver and family we meet. Our services include:

Who Can Refer?
Anyone can request Hospice services with a physician's order and verification that there is an illness with a life-limiting prognosis. Care is available for patients with a prognosis of six months or less, such as patients with terminal cancer, end-stage heart disease, ALS, chronic obstructive pulmonary disease, renal failure, AIDS and dementia. Patients do not need to be homebound in order to receive Hospice care.

What is the Cost of Hospice?
The Medicare Hospice benefit covers all services, medications, supplies and equipment related to the terminal illness. Other insurances may offer some coverage for Hospice, but may not cover the entire cost. If a patient has no insurance or partial coverage, we will work with each patient on an individual basis.
