If you have a suggestion or concern regarding our services or your medical care, please notify our staff and they will assist you. The Customer Service Patient Request Line is available 24 hours a day to resolve concerns regarding your room, food, or nursing care. If you prefer, you may contact the Patient Representative during business hours Monday through Friday, by calling 586-573-5696. If attempts to resolve the concern are not acceptable to you or your family, you may obtain a copy of the "Citizens Guide for Filing a Complaint Against a Health Facility" which is published by the State of Michigan, Department of Consumer and Industry Services, Bureau of Health systems. This pamphlet explains how to file a formal complaint to the MDCIS and is available in public areas throughout the hospital. You may also receive a copy by contacting the Patient Representative at 586-573-5696 or by calling MCDIS at 1-800-882-6006. St. John Providence Health System hospitals are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). If your concerns cannot be resolved through the hospital, call JCAHO at 1-800-994-6610, 9:30 a.m. to 6 p.m. Monday-Friday. Financial AssistanceSt. John Providence Health System Hospitals have programs to assist patients who are uninsured or cannot afford medical care. Our programs include sliding scale discounts, financial assistance for those who meet income requirements and referrals to federally funded programs. However, you should remember that your policy is a contract between you and your insurance company, and you have the final responsibility for payment of your hospital bill. We have several payment options available to assist you in paying your bill. Spiritual Care ServicesSt. John Macomb-Oakland Hospital, Macomb Center, has a chaplain and spiritual care volunteers who can provide spiritual and emotional guidance to patients and families of all religious denominations. Patients can also receive visits from their own clergy. Our staff can assist in contacting the patient's own clergy. Ethics and Health CareEthics is a system of moral principles and/or rules of conduct that are recognized by and associated with a certain group or culture. St. John Macomb-Oakland Hospital, Macomb Center, is committed to providing health care that meets the highest of ethical standards. Because complex ethical questions sometimes arise in a hospital setting, the hospital’s Ethics Committee was established to support and advise patients, families and caregivers as they work together to find solutions to difficult problems. The Ethics Committee is a multidisciplinary group made up of representatives from nursing, physicians, clergy and other staff. All members of the Ethics Committee have received education in addressing critical issues. Patients, families, physicians and hospital staff may request the Ethics Committee review a particular case. To request an Ethics Committee consult, please contact any nurse, physician, or manager or call the Clinical Resource Management Manager at 586-573-5204 or 586-573-5010 Your Hospital Stay and What You Will NeedDuring your hospital stay, you only need to bring a few personal items to the hospital such as bathrobe, slippers, reading materials and personal toiletries. The hospital provides soap, bed and bath linens. Toothbrush, toothpaste, comb, lotion and other personal items are available upon request. Personal Belongings: Do not bring jewelry, money, cell phones or other valuables. The hospital is not an insurer of patient property and cannot be responsible for lost items. St. John Macomb-Oakland Hospital, Macomb Center, discourages the use of personally owned electrical equipment such as hair dryers, electric razors, radios and the like. For your safety the Clinical Engineering department, prior to use, must examine any electrical devices brought into the hospital. Lost and Found: Unclaimed articles are turned into the Environmental Services department where they are kept for 90 days. To inquire about lost articles, call Environmental Services staff at 586-573-5273. Birth CertificateTo obtain a birth certificate for babies born at St. John Macomb-Oakland Hospital, Macomb Center, call the Warren City Clerk's office at 586-574-4557 or visit the Warren City Clerk web site. Medical RecordsIf you need a copy of your medical records, print and complete the Release of Medical Records Authorization form. Be sure to specify the dates of service and the type of information needed. If you are requesting records sent to you or to a physician outside of St. John Health, you will receive a bill.
