Written doctors order and referral or authorization if required - some insurance companies require pre-authorization, if you present for your test/procedure without pre-authorization you may need to be rescheduled.

Any insurance co-pays are expected at time of service. We will also request payment for any outstanding balance you may have. We accept check, cash, Visa and MasterCard.
