Social Work services are provided for any diagnosis or psychosocial crisis resulting from the stresses of disease on the individual patient and their family. Social workers serve our patients by making the transition from home to hospital smoother by utilizing their assessment skills to identify problem areas. Social Work Services may also be ordered by a physician for a homebound patient. A personalized treatment plan is then created to facilitate healing and provide a safer care environment with support for the patient. Social workers are available with pediatric, adult and geriatric skills.

• Services provided include:
• Community referrals for private hire of personal care, housekeeping, meals on wheels, transportation, adult daycare or housing services available in the community.
• Long range planning for nursing home placement or hospice services.
• Short-term therapy to increase the patient/family understanding of disease, increased hope and sense of control, increased self-esteem and body image, improved patient compliance with the medical regime, decrease anxiety and depression, relaxation techniques.
• Crisis management of abuse/neglect, inadequate food/medical supplies, dual patients in one household, suicidal thinking.
• Direction with financial crisis through medication assistance, obtaining medical equipment, application for Medicaid and chore grants.
• Legal referrals for social security benefits like SSI/SSDI, Veterans benefits.
• Assistance with completion of the Medical Durable Power of Attorney, information on Conservatorship or Guardianship.
• Psychosocial education through support groups, behavioral plans, caregiver support.
