Infusion Services are provided by registered nurses and pharmacists who work directly with the patient’s physician to develop a patient specific treatment plan. Our Infusion Services program provides the quality care you expect from St. John.

Comprehensive Patient Support Services
Pre-Discharge Services
- Evaluation of treatment plan and equipment needs
- Insurance verification
- Investigation of alternative financial arrangements for therapy
- Predischarge instruction with the patient and/or caregiver
- Coordination of care with physicians and home care nurse
- Home delivery of medications and supplies
- Pharmaceutical care planning
Post-Discharge Services
- Routine contact with the patient regarding treatment process, complications, and supply needs
- Regular lab monitoring
- Drug information and education
- 24-hour on-call service
- Coordination of therapy changes
Criteria for a Home Infusion Patient
- Diagnosis consistent with IV Therapy.
- Patient is determined to be medically stable by the referring physician.
- Patient and/or caregiver has the ability to learn the procedures.
- Patient and/or caregiver has good motivation to learn the procedures.
- Patient has received the initial dose of prescribed medication without unfavorable effects and has satisfactory venous access.
